Apologies for never concluding this thread!
After re-visiting a clutch specialist, we were advised the clutch plate was faulty (perhaps incorrect advice?).
Purchased a new Xtreme Heavy Duty clutch, with same brand lightweight flywheel.
Installed with no issues and has been functioning flawlessly since.
