It's just a generic Narva one, the plug is the same but the clip that holds it onto the car is different.

Just bring the old one in and they should be able to just match the plugs to one on the shelf. (sorry, I threw the package out so can't give you a art number)

I ended up at super cheap. I tried Repco, but they had no stock. Super cheap's one was $21.99 and is manufactured by Tridon.

In terms of fitment, I took both the old and new casings apart. The PCB on the new unit is a touch larger than the old one, so I shoehorned it in to the old box. There's a bit of an air gap between the cover and the casing, so I just taped around it with electrical tape. At least I can use the factory clips and mounts that way. Hopefully the tape will hold for long enough. At least if it doesn't, I've figured out the easy way of getting to the relay box. It does require going off feel, but it's not too difficult once you've figured it out.
