It appears to be screwed on but it is so tight that I am afraid I will have to apply so much force that I will break something in trying to take it off. There seems to be what feels like a lock nut at the base of the knob, but it is covered by a spat or gaiter that I can't remove without tearing it. What do I do?

THey are just screwed on really right.

Get a thick rag and ocver the gear knob with it.

Next gewt a set of multi grips/stilsons/ monkey wrench and rip away at it (the rag stops any damage to gear knob).

Off she comes - I have used this method a number of times - Just be careful not to damage the knob.
