Yup, you need one hole in the inner ring for sequential. See here: linky.
Also, is the disc the right way up? ie you haven't flipped it? Although that wouldn't stop spark (seeing as spark is the outer ring).
If you manually rotate the CAS with it out of the car, do any ignition/injection events occur?

Injectronics make new CAS inserts which you can put in your old CAS when you kill them. $220 new - takes about 10 minutes to fit. Just in case you wanted to look into it......... I know for a fact they make 3 B6/BP/GTX models all which are slightly different.
